Converting BenefitsIt is very important that you inform the terminating (or retiring) employee of the right to convert benefits. When employment terminates due to retirement, severance, etc., the employee should be advised that all or part of their life insurance may be converted to an individual policy without medical evidence, provided an application is made within 31 days of the formal termination date. The employee will be required to contact the nearest The Co-operators’ office in order to obtain the required life insurance conversion documents. The employee is also eligible to convert extended health and dental care benefits to an individual policy within 60 days of termination under the group plan. Please note that the individual policies do not provide coverage that is identical to the group plan. An interested employee can find information about the health and dental conversion in the Morneau Conversion Program brochure. Should the employee decide to apply for the health and dental conversion, he/she will be required to complete the Morneau Conversion Program application.
