Adding or Deleting Dependents

1. Plan Administrator completes a Group Insurance Change Form by indicating the employee's social insurance number, name, effective date of change and only the information that is changing (i.e., single to family or family to single). To ensure that this change is made on your next monthly premium statement, submit it to Morneau Sobeco prior to the 15th of the month. This change form may either be mailed with your premium remittance or faxed to (416) 445-7989.

Note: If the employee is adding a new dependent when he/she already has family coverage, the Group Insurance Change Form is not required. The monthly premium statement will not change as he/she is already paying premiums for family coverage. When the employee submits his/her first claim for the new dependent, The Co-operators' will add the dependent to their claims payment system.
